I-TRANSFER is an implementation science study focused on improving identification of sepsis survivors, care transitions to home health and outpatient care, and timely attention the first week after discharge. This session will report on the barriers, facilitators, implementation strategies, and outcomes of the I-TRANSFER study across five health systems with a special focus on documentation of sepsis and information transfer to the post-acute care. 

Session Objectives:

  • List the components of the I-TRANSFER protocol and its impact on documentation, post-acute visit patterns, and patient outcomes. 
  • Discuss the common barriers to clear communication of sepsis diagnosis to the next level of care. 
  • Analyze which barriers to optimal sepsis documentation and information transfer exist and which implementation strategies would be feasible in your health system.
