Addressing the Need for Cross-Continuum Care Collaboration

Overview

This webinar will highlight the struggles related to poor cross-continuum collaboration and the need for greater connectivity between acute and post-acute care settings. The key to resolution lies in data-sharing and interoperability. The discussion will focus on the nuances of interoperability and enhancing collaboration across the continuum and offer best practices to better communicate and share data across the health care ecosystem.

Additionally, Pennsylvania health systems continue to struggle with ED throughput. It is more important than ever for hospitals and post-acute providers to prioritize collaboration across care settings as a foundational pillar to succinctly combat this issue.
